Thank you for your interest in joining Towerbank After School Club.

You can apply now using this form for places in the next academic year. We cannot guarantee that you will get a place at this stage as we are usually over-subscribed. We follow our allocations policy to be sure we are as fair as possible. If you are unsuccessful at this time you will be added to our waiting list and you will be informed when a place becomes free. There is often movement in August or September so we hope you will not have to wait too long.

Please fill out the following form to give us the information we need to offer your child a space. As part of the form we will require you to upload a photograph of your child, so please have ready a recognisable, recent, clear, head & shoulders shot of your child.

You will also be asked to pay your membership fee of £20 (your child will not be offered a place at TASC without it. Refundable if you choose to be removed from the waiting list) by Bank Transfer so it will speed up your application to get that ready too.

Places will be allocated based on the time stamp of your application, in the order of priority based on our TASC Allocations Policy. Please allow 28 days for your application to be processed.

Permissions: Outings, Sun Protection and Sharing Information with Towerbank School

As a regular part of your child’s involvement with the Club they may take part in a range of activities outwith the Club’s premises, e.g. walks, visits to local parks, beach, museums and cinema. The outing may involve travel by public transport or coach.I agree that my child can take part in the regular programme of activities organised and supervised by staff at the Club.

I give permission for my child/children to watch PG films deemed suitable by staff at the Club for viewing with the group of children present.

TASC is concerned about protecting your child from sunburn and skin damage.As Parent/Carer I will provide a suitable hat, such as a legionnaire’s hat or sunhat on sunny days and pre-apply sunscreen to any exposed parts.

With your consent we will also help your child apply sunscreen when necessary (based on UV index checks)I am happy for my child to have sunscreen applied at TASC.

I am happy for my child to use the sunscreen provided by the Club.

If not I will provide a bottle of sunscreen labelled with my child’s name for use at the Club.

We take a holistic approach to caring for your child. From time to time it can be helpful to us to share information with school management, class teachers, learning assistants or other staff at Towerbank School.I give permission for TASC to confidentially, appropriately and sensitively share information/ discuss the needs of my child with relevant staff at Towerbank School
